By filling out the previous form, you are consenting to provide HOT Cleaning Services with information that may be “sensitive information”, a category of personal information under the Privacy Act. This information is currently being collected for the purpose of ensuring the safety of HOT Cleaning Services employees and other members of the community by minimising the transmission of the COVID19 virus. The information will be used to assist HOT Cleaning Services to determine whether its staff can safely carry out the cleaning services and to ensure that preventative and protective measures are taken when necessary. We do not require you to provide the names or any other identifying information of any of the people who reside with you or with whom you have had physical contact. The information is collected for HOT Cleaning Services internal decision-making and planning purposes only and will not be disclosed, other than to Senior Operations Staff and the Directors or where required by law. HOT Cleaning Services records of the information will be kept on a secure server only accessible by Senior Operations Staff and or the Director and will be destroyed/deleted once the Australian government advises that the existence and transmission of COVID19 is no longer a public health emergency.
